Athletic Clearance

IN ORDER TO PARTICIPATE IN WILSON ATHLETICS, A STUDENT MUST COMPLETE THE ONLINE CLEARANCE PROCESS.

Athletes participating in summer camps must complete the above forms and submit to their coach prior to participation.

PHYSICAL EXAM

Students must have a physical examination by a medical doctor.

The completed physical examination form must be uploaded when completing the online clearance process and submitted to the Athletic Secretary, Denise Cuevas in the main office.

Suggested Locations for Sports Physical

A physical is valid for one year from the date of the exam.

 

MEDICAL INSURANCE REQUIRED

In order to participate in athletics, all student-athletes must have valid medical insurance coverage. If you do not have current medical insurance, you may purchase a plan through Myers-Steven Insurance. Please see the Athletic Secretary in the main office to pick up a form.
